After eight years of dedicated service, Advanced Vein Institute of Arizona will be closing its doors. Our last day seeing patients will be November 30, 2024.
We want to extend our heartfelt gratitude to each of you for trusting us with your health and for allowing us to be a part of your lives. Since 2016, it has been our privilege to provide compassionate, expert care to help improve the quality of life for our patients.

Request Medical Records
As we close our practice, we want to ensure that you have access to your medical records. If you need copies of your medical records, please submit a request by email or fax (480) 868-2272 or before 11/30/2024 you can reach us by phone (480) 454-5562. We are dedicated to providing these records in a timely manner and will work with you to send them directly to you or to another healthcare provider of your choice.
Please make your request as soon as possible to facilitate the transfer of your information. If you have any questions about the process or need assistance, feel free to reach out to us.
